This Sample: Interview Assessment Form can be used to record applicant responses during a structured interview.

A structured interview ensures that all applicants are interviewed in the same manner with the same questions asked, which allows each candidate the same opportunity to prove they are the best person for the job.

Asking the same questions of each applicant will also allow effective comparisons between candidates and reduces the possibility of |bias or discrimination.  To assist in decision making, a rating system can also be used to more objectively assess the quality of the responses against the key selection criteria.

Interview questions should be linked to the key selection criteria and adhere to a competency-based interview framework where behavioural responses can be provided i.e. how they would undertake tasks. As such, candidates should be asked to provide examples, including a description of the scenario, what they did or how they undertook the task and what the outcome was.

Interviewers must take care when asking questions to ensure that they are down professionally and in a manner that does not breach legislation or generate risk. Applicants should be encouraged to take their time and to provide as much detail as needed for the panel to make an accurate assessment. Each interview panel member should be provided with one assessment form per candidate for completion.  These can be collated following completion of interviews using the Interview Scoring Matrix Form. Interviews should complete their own form prior to discussing the candidates to avoid ‘group think’.
